I fully understand that this is an optional activity for my child. Parents must treat coaches, players, and referees with respect. In consideration of you accepting my child’s entry, I hereby assume for myself/my child, all risks for damages I/my child may have against the LCV Youth Tackle Football League or its volunteers connected with the program.

I also acknowledge for myself/my child that LCV Youth Tackle Football League provides no medical coverage of any kind for any accidents or injuries that might result during participation in LCV Youth Tackle Football League activities.
